Thomas J. Long School of Pharmacy Fellowships
Pharmacy Practice Fellowship

This two-year fellowship is designed for individuals interested in academia in advanced pharmacy practice and who desire to acquire extensive experience in ambulatory care practice and clinical research. The well-rounded training and experience gained through this program equips graduates to be successful in academic or ambulatory care practice. Fellows will also obtain formal training to be an independent researcher by joining multiple research projects.

Areas of experience

The rotation options include, but are not limited to:

  • Medicare Part D/geriatric outreach
  • Acute inpatient medicine
  • Ambulatory care practice (including diabetes, heart failure, anticoagulation management and transitions of care)
  • Education methodology and assessment

Clinical rotations will be offered in internal medicine and ambulatory care, mostly occurring at the county teaching institution San Joaquin General Hospital. Additional experience in diabetes, hypertension and hyperlipidemia management will be acquired at the Community Medical Center in Stockton. The fellows’ participation in the ambulatory clinics spans the duration of the fellowship program and includes pharmacist-run and collaborative practice clinics. Through their longitudinal ambulatory care experience, the fellows will acquire sufficient experience to become certified as an Advanced Practice Pharmacist.

Gain valuable experience by engaging in unique public health outreach

Through their public health rotation in the one-of-a-kind Medicare Part D Outreach Clinics at University of the Pacific’s Thomas J. Long School of Pharmacy, the fellows have the opportunity to gain expertise in the Medicare benefit and serve as preceptors during health care outreach events hosted in the San Joaquin and Stanislaus Counties. Specializing in this niche topic dovetails perfectly with serving the patient population at San Joaquin General Hospital.

Dive into the world of academia

Most importantly, fellows will obtain a wide spectrum of teaching experiences as a clinical assistant professor. Through this role, fellows will experience traditional didactic large classroom teaching, case-study leading and experiential precepting. The variety of teaching experiences acquired by the fellows will allow them to practice as faculty in didactic and experiential settings.

The fellows can leverage their involvements in both academic and clinical settings to join various research projects, working alongside university faculty and health care professionals. They will gain first-hand experience in the comprehensive process of conducting research, with the opportunity to publish novel findings at the completion of their projects.

Alumna Allison Mac

“My time as a pharmacy practice fellow at Pacific was incredibly rewarding. The fellowship provided invaluable opportunities to refine my skills in patient care, research and academia, all while working alongside highly supportive and encouraging faculty.

One of the most fulfilling aspects was engaging in the teaching and mentorship of students. I loved getting to know my students, witnessing their growth and celebrating their accomplishments.

The fellowship’s comprehensive training empowered me to thrive as a confident and capable pharmacist, ready to tackle any challenge.”

Pharmacy fellow Tiffany Vu

“Being a Pacific Pharmacy Practice Fellow has allowed me to learn and grow confidence and skills to be a teacher, leader and health care provider. The 2-year program allows you to work with clinicians, faculty and patients in a more longitudinal practice. What sets this program apart are the unique opportunities and supportive faculty that foster an environment that allows me to challenge myself and gain a foundational background to establish a career in whatever piques your interest, including ambulatory care, academia and research.

Similar to how the Thomas J. Long School of Pharmacy constantly evolves with the pharmacy practice, this program evolves with fellows’ interests and feedback, creating a tailored curriculum that always keeps you on your toes. The variety of experiences in all these aspects helped deepen my knowledge of pharmacy practice and provided me with skills that I will take with me beyond the 2 years I've spent here.”

Pharmacy alumnus Derek Huang

“Pursuing a fellowship in both academia and ambulatory care, with a focus on diabetes and congestive heart failure (CHF), has been one of the most transformative experiences of my professional journey. The integration of hands-on patient care and a robust academic foundation has not only sharpened my clinical skills but also deepened my understanding of patient-centered care.

Throughout the fellowship, I have worked closely with patients managing chronic conditions such as diabetes and CHF, gaining invaluable experience in optimizing medication therapy, addressing barriers to care and improving adherence to treatment plans. These experiences, guided by expert preceptors, have solidified my commitment to ambulatory care and underscored the vital role pharmacists play in improving health outcomes.

The academic component of the fellowship has been equally enriching. Engaging in teaching and mentoring has allowed me to bridge clinical practice and academia, offering a unique opportunity to contribute to pharmacy education while honing my expertise in disease management. This dual focus has empowered me to grow both as a clinician and an educator, reinforcing my dedication to advancing the profession of pharmacy.”
